To convert total outstanding sewerage bills into easy instalment payment

To be completed by payee (tenant/owner) who is responsible to pay for the sewerage bills

To request excess payment to be refunded to the rightful payee

 

Letter of Indemnity Form

To be completed by receiver

 

Letter of Authority & Discharge Form

To be completed by owner / payee if the receiver is not the same owner /payee

 

Other supporting requirement for Request for Refund

  1. Copy of NRIC

  2. Bank Account Details

  3. Copy of Payment proof with payment details

  4. Form 13 and Form 49 (applicable for company)

Transfer Credit Payment Form

To be completed by payee to request transfer payment from one sewerage account to another sewerage account

Other supporting requirement for Request of Payment Transfer

  1. Copy of Payment proof with payment details

 

Declaration Form

To be completed by premise owners or occupier on semi-annually in order to qualify for an industrial billing rate.
